A valid health card is required. Red and White health cards are no longer valid, as per Service Ontario. We do not accept third party insurance. If you do not have a valid health card, you will have to pay for your appointment up front and you can submit your receipt afterwards for reimbursement.
This is for a ONE STEP TB Skin Test. This fee includes the injection, reading, and a note to verify status, and is completed over two appointments two days apart. (ie Injection on Tuesday, read on Thursday) If you require a TWO STEP TB Skin Test, the same process and fees are repeated 14 days after the first injection.
This includes physicals for school, work, sports, and extracurricular activities. This does not include any chart copies included in the request.
This includes various types of forms requested or required by your insurance company. Please read your forms thoroughly - most will say whether the fees are your responsibility or your insurance company's responsibility. This does not include any chart copies included in the request.
This is for a replacement of your yellow immunization card. Alternatively, we can print what we have available in your chart for a smaller fee. Please note we only have immediate access to records 2019-Present.
You can also obtain immunization records from the Thunder Bay District Health Unit for free - we forward all our records to them immediately after immunization appointments.
As per the OMA, chart copies are $30 for the first 20 pages, and then $0.25 per page after that.
